The situation

The procedure changed. The clipboard cannot say who read it.

A two-plant food processor revises its lockout procedure after a near miss on a slicer. The change goes out at three shift huddles, on the safety board and in a group text. A month later the inspector asks for training records with each employee's name and dates. The EHS manager has a sign-in sheet from one shift and a photo of the board. The plants employ 380 people.
